Physicians registration (in-person workshops)

Before completing and submitting the form, please carefully read the following points:

1- Your registration will be processed on the website immediately after registration. If you decide to withdraw from the workshop, 20% of the deposited amount will be deducted and the rest will be refunded up to 24 hours before the workshop starts. After this time, 50% of the deposited amount will be deducted and the rest will be refunded until the start of the workshop. There will be no possibility of refunding the tuition fee after the workshop has started.

2- In case of cancellation of registration, in addition to the cancellation penalty mentioned in point 1, the applicant will not be able to register for the desired workshop in future sessions.

3- If for any reason – including incorrect address and postal code, the certificate returned from Biocare Europe due to the absence of the certificate holder at the time of postal delivery, the applicant is obliged to pay the postage fee for re-sending the certificate.

4- Certificates will be issued within a minimum of 3 months of European working time and will be sent to the participants.

I, as an applicant to participate in educational workshops personally and at my request in the Pasargad Medical Group, solemnly commit with full mental health:
- I have participated in the relevant educational workshops with full willingness and complete awareness based on personal interest.
- I will perform all assigned duties in the course after receiving the necessary training to the best of my ability, with care and in accordance with the law.
- After completing the course, I have no right or claim to employment in the Pasargad Medical Group.
- The issuance of a complete certificate of the training course by the Pasargad Medical Group to me is solely subject to the complete completion of the course and the approval of the course instructor. The certificate received will not entitle me to future activity.
- I hereby personally take responsibility for any learning difficulties from the beginning to the end of the training course and any future activities that I become aware of, and acknowledge that the Pasargad Medical Group has no commitment in this regard.
